I have now fully decoded the embedded metadata in the test image I linked in a previous post. It only has level 1 data though, but I believe I can also decode all the other levels. Bumping @DMDreview @djnice and anyone else that can provide captured data of tv-led DV from a known sample that has as many possible levels to confirm the decoding.
